When a hospital or clinic is to be registered for membership with ZACH the following documents are required to accompany the application letter addressed to the Management Board Chairperson:
- Organisation /Church profile, Organogram, Board Membership list
- PVO Registration Certificate
- Health Professions Authority Registration Certificate
- Ministry of Health Registration Certificate
- Practicing Certificate of Medical Doctor/SIC at the hospital/clinic
